/year
and only $477.00 for each additional family member
2 Free simple cleanings/year or Periodontal cleaning Copay $35/per visit (up to two per year)
2 Free periodic exam/year
Free x-rays
1 Free fluoride treatment per year
Free take home teeth whitening, custom trays initially, thereafter 2 whitening tubes per year
20% discount on all dental procedures
Emergency exams are $30 per appointment, and will be waived if treatment is completed the same day
Eligible family members include spouse and dependant children under the age 19 (up to 26 if the dependant child if a full-time student). All plan membership fees are payable in full at the time of registration and are non-refundable. Plan duration is for one year from the registration date. A missed appointment fee of 25% of treatment total will be charged for all missed dental appointments. Please notify our office 48 hours in advance, if you must change a scheduled appointment. .

What forms of payment are accepted?
All forms of credit and debit cards including Visa, Mastercard, Discover and American Express.
When can I start using my membership?
Your membership is effective immediately!
How do I schedule my preventive care?
Simply call our office and set up an appointment for yourself or your family members.!
How can I use my Member Exclusive discount for restorative, elective or cosmetic treatment?
Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is “current” and you will be eligible for instant savings off our regular fees.
How do I know if I need Perio membership?
If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.
What happens if I miss an appointment?
It will be your responsibility to reschedule. Please see our office policy regarding no-shows and last minute cancellations.
Can I add members to my account?
Absolutely! Your entire family can join.
Can I transfer my membership to another dentist?
Your club membership is solely with our office and as such is non-transferrable. If you move to another area, you may cancel your membership at any time.
How can I check on the status of my account?
Login to your Member Portal and look under Account Members. You’ll see the status of each member above his/her name.
Can I change or update my billing information?
Yes! Simply login to your Member Portal and click “Edit Your Payment Method”.
How do I cancel my membership?
Login to your Member Portal and click the “Remove” button for each member you’d like to cancel.
If I cancel, can I get a refund on my membership?
No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.
How much do I pay to sign up for my club membership?
You’ll pay only the Lifetime Activation fee to join. This fee includes your first month’s membership dues.
Can I prepay my membership fees?
Memberships are currently month-to-month only.
What information is required for me to sign up for my club membership?
An email address for receipts, credit or debit card, full name, address, phone and date of birth.

The team will take the time to get to know you and understand your dental health needs, preferences, and goals.
Exam
We will perform a comprehensive exam and walk you through his findings. We'll take any necessary x-rays to determine whether there are any hidden dental health issues that should be addressed.
Professionally Clean
Our team will professionally clean your teeth and gums, removing any plaque and bacteria from hard-to-reach places that can’t be eliminated with a toothbrush or floss.
Schedule Next Visit
If there’s any need for a follow-up appointment, we’ll schedule your next visit at a time that works best for you. If you’re given a clean bill of health, we’ll simply book your next visit in six months!

Meet Dr. Tracey Osborn Pike
Dr. Tracey Osborn Pike is a New Hampshire native, born and raised in Londonderry. Growing up, Dr. Pike loved to spend time riding and showing horses…and even showed some of them at the Deerfield Fair! After high school, she left New Hampshire for Clark University to study biology and chemistry. She then went on to Tufts University, School of Dental Medicine and graduated in 2003. She moved to beautiful Deerfield and opened Deerfield Family Dentistry.
